How it Works
Intake Form
Complete our intake form and complete all of the required paperwork required to set up a consultation with a prescriber
Consultation
Meet with a prescriber virtually or in-person. Consultations typically last anywhere from 10 - 30 minutes.
Receive Your Kit
Once you have completed all paperwork and met with a prescriber, your kit will be available at Kitch Pharmacy.
